Our first concern is to make sure that our fees for each and every service we offer are affordable and reflective of the high quality of service our patients deserve and expect. Secondly, we will assist you in obtaining optimum benefits from your insurance plan. We have the capability of submitting both dental and medical insurance claims. As a courtesy to our patients, we will file most primary and secondary insurance claims at no cost to our patients. Any estimated patient co-pays and deductibles are payable by cash, check or credit card. We accept Visa, MasterCard, American Express and Discover Card. Dr. Horner is a PPO Dental Provider for Blue Cross Blue Shield of Alabama, United Concordia and Delta Dental Premier Insurance.
Because some dental procedures/treatments such as TMD & Sleep Apnea Oral Appliance Therapy (OAT) may be covered by medical insurance, we ask our patients to provide their insurance information for both medical and dental, so maximum benefits can be obtained for you, the patient.
